At The Puzzle Box, we take your family’s privacy seriously. Federal and state laws require us to safeguard all information that could identify you or your child. This includes names, addresses, insurance details, and any health-related information connected to evaluations, treatment, or progress. This type of information is known as Protected Health Information (PHI).

This notice explains how we protect your information, the ways we may use or share it, and what rights you have regarding your records.

Changes to This Policy

From time to time, laws or our practices may change. If that happens, we will update this notice to reflect the changes. Updated copies will be available in your parent portal and can also be provided in paper form if requested. You may ask for a copy of our most current privacy practices at any time. Families will also be asked to sign a form acknowledging receipt of this notice.

When We May Use or Share Your Information Without Extra Permission

In order to provide services, there are a few situations where we may use or share your information without needing a separate authorization from you:

• For Treatment: Members of your child’s care team may access and share necessary information so that services are coordinated effectively.
• For Payment: We may provide health details to your insurance company or funding source to arrange payment for evaluations, therapy sessions, or progress reviews.
• For Limited, Non-Identifying Use: Materials such as thank you notes, stories, or photos shared with us may be used in ways that do not reveal personal identity (for example, for mission-related outreach or community education). Identifying details will never be shared without your explicit written consent.

Your Role in Protecting Privacy

You may request limits on how we use or share your child’s health information. While we are not always required to accept every restriction, if we do agree, we will honor that agreement except when disclosure is required by law or necessary in an emergency. Under HIPAA, you also have the right to restrict information from being shared with an insurance plan if you choose to pay for those services out of pocket.

You may also choose to provide written permission to share information in other ways beyond those described above. This could include sharing records with another provider, a school, or a family advocate. You are in control of these decisions, and you can change or revoke permissions at any time.
